I found a 2005 Oregon quarter with what looks like a dime blank in the middle and the mirror image of the head on the heads side that was struck over the original head and a head strike on the tails side. How could this have happened at the mint?

Hi, Larry. What you have is a coin that had another glued to it. When they were separated, you coin retained the glue with the imprint of the separated coin. This was done outside of the mint. As others here have said, WE NEED PICTURES!
