Any thoughts on whether this is an error or damage? I haven't been able to find anything quite like it through internet search.

Based on the obverse, I'd have said there was a chance this could be a lamination, but seeing the reverse, I'm not so sure. It could just be damage, or perhaps a planchet flaw. I'll defer to those with more specialized experience in error coins for that answer.
It looks to me like the Upraised Rim was pushed down on both sides. Can you post a large picture of the Edge especially near the date?
@Gatorsuze Thanks for posting the enlargements. I don't know why @Peter T Davis doesn't get rid of the "Thumbnail" option because they are useless. To me, it looks like the coin was damaged after it left the Mint. It looks like something was used to grip the coin, and it sheared part of the metal away from the rim. The metal remained intact, but the coin continued to circulate over many years which wore it down. Of course, this is all speculation on my part, and we will probably never know what really caused it. Chris
